What is shockwave therapy?

Shockwave therapy or extracorporeal shockwave therapy (ESWT) is an innovative,

cost-effective, evidence-based treatment for tendon pain (often known as tendinitis

or tendinopathy) and other conditions throughout the body. Extracorporeal means

outside of the body.

The main benefits of shockwave are pain relief and restoration of function. Shockwave

therapy has been shown to be an effective treatment modality for a range of common

tendon complaints including plantar fasciitis, Achilles tendinopathy and tennis elbow.

Shockwave therapy is a safe, non-invasive treatment with convincing evidence for its

effectiveness from a large body of research and clinical trials. There are now multiple,

randomised, double-blinded clinical trials that support the use of shockwave therapy

for plantar fasciitis, lateral epicondylitis, and calcific tendinitis of the shoulder.

What is shockwave therapy used for?
Shockwave therapy is an effective treatment modality for a variety of lower and upper

limb conditions. These include:

Bone injuries – Shin splints (MTSS), stress fractures and non-union fractures
Calcific tendinopathy
Tennis elbow (common extensor tendinopathy)
Golfer’s elbow (common flexor tendinopathy)
Greater trochanteric pain syndrome (also referred to as gluteus medius tendinopathy

and/or trochanteric bursitis)
Proximal Hamstring Tendinopathy (also referred to as high hamstring tendinopathy,

or hamstring origin tendinopathy)
Patella tendinopathy
Insertional Achilles tendinopathy
Mid-portion Achilles tendinopathy
Plantar fasciitis

How does shockwave therapy work?
This is sometimes known as its ‘mechanism of action’. Shockwave therapy uses a specialist

device to deliver acoustic energy through the skin to the injured part of the body. The

shockwaves are mechanical and not electric. They are audible, low-energy sound waves,

which increase the blood flow to the injured area.

Shockwave therapy has two main modes of action which will help with persistent tendon

pain. First the shockwaves work to desensitise nerve endings which will often give an

immediate reduction in pain. Secondly, and often most importantly, the shockwaves

cause controlled micro-trauma (microscopic damage) to the tissues which encourages

the body to respond by increasing the blood circulation and metabolism in the affected

area. This will activate and accelerate the body’s own healing response. The

shockwaves can also ‘breakdown’ disorganised tissue and calcifications.

Principle

Shockwave with high energy peak used in Shockwave therapy interact with tissue causing overall medical effects of accelerated
tissue repair and cell growth, analgesia and mobility restoration. All the processes mentioned in this section are typically
employed simultaneously and are used to treat chronic, sub-acute and acute (advanced users only) conditions.

The Shockwave machine low-energy defocused extracorporeal generated shock waves, mainly for physical therapy treatment, sports
injury and rehabitation and ed treatment.

How radial ESWT works

Compressed air accelerates a projectile up to 80 to 90 kph within a guiding tube that strikes a metal

applicator placed on the patient's skin. The kinetic energy is converted into a radial shock wave. The

projectile generates stress waves in the applicator that transmit pressure waves into tissue to a depth of 4 to 5 cm.

The evolving list of diagnoses for which ESWT shows potential includes:

Shoulder tendinopathy
Lateral epicondylosis*/epicondylitis
Greater trochanteric pain syndrome
Hamstring tendinopathy
Patellar tendinopathy
Medial tibial stress syndrome
Achilles tendinopathy
Plantar fasciitis*
Knee osteoarthritis
Fracture nonunion

Spasticity of spinal and supraspinal origin (stroke, brain injury, multiple sclerosis, cerebral palsy)
*Device approved by the FDA but not covered by most insurance plans

What is the history of Shockwave Therapy?

Long story short – Shockwave therapy was first created in the 1970’s and was initially used as

a treatment method for kidney stones. Using this method meant more and more people were

avoiding the need for a painful, invasive surgery, and the same goes for musculo-skeletal conditions today.

Is Shockwave Therapy clinically proven?

Shockwave Therapy is clinically proven, recognised by the top orthopaedic hospitals and used

by high profile professional sports bodies such as the English Institute of Sport, UK Athletics,

Welsh Rugby Union and many Premiere League football clubs.

What are Shockwaves?

Shock waves are high-energy, audible sound waves. In medicine, shock waves have been used

since 1980 among other things for the dissolution of kidney stones. In rehabilitation therapy,

shockwaves are transmitted to the area to accelerate healing. SHOCKWAVE THERAPY (ESWT)

involves passing short, intense sound waves into affected tissues, which increases blood flow,

has an analgesic effect and stimulates repair and regeneration. The treatment puts the cells into

repair mode and allows your body to restart its own accelerated healing process.

Shockwave Therapy ESWT safe?
Yes. It has NICE guidance and FDA approval for a large number of indications. shockwave Therapy

performed by qualified therapists is a largely risk-free and side-effect-free method of treating

musculo-skeletal conditions and chronic pain.
